Daybreak Member Application

Please print and send the completed application to Aaron-Marie Thoms, Membership Chair, via fax (319-365-6919), scan and email (athoms@millhisersmith.com) or snail mail (Aaron-Marie Thoms, c/o Millhiser Smith Agency, Inc., PO Box 3100, Cedar Rapids. IA  52406-3100).

Once the application has been received, you will be contacted with club information and membership details.

During the application/new member phase, please attend regular Daybreak Rotary meetings, as you will be introduced to the club several times before formal induction.
